What we need to do here is make the replacement

       MACRO_NAME => MACRO\_NAME

However, this replacement should be applied only to bare text -- text not between $ ... $
or [ ... ]. @otavo, would you like to do this one?

I am under heavy load in two customer projects which involve live video streaming in an i.MX6 embedded platform. No free time for the coming weeks, I am sorry @jxxcarlson.

This is a similar problem that the manpage backend has in that content has to be "manified" (in this case latexified). While it's possible to hack in a solution in the short term, we really need to have a processing step in Asciidoctor that prepares text for the target medium. We actually do some of this inside of Asciidoctor already, but it assumes it is outputting to *ML. My guess is that we need to overload the "specialchars" substitution so that different backend can define what exactly "specialchars" means and what should be done. I've added a note to my worklog to look into this.
